Grant Funding to Expand Access to Primary Care in Rural Maryland
Funding Opportunity
The MHCC is pleased to announce the release of a Request for Applications (RFA) for a funding opportunity to support the development and expansion of advanced primary care practices within designated rural areas of Maryland. Eligible organizations include primary care practices, Federally Qualified Health Centers, and organizations prepared to sponsor an advanced primary care practice
Who is Eligible
The MHCC is seeking applications from eligible organizations, including primary care practices, Federally Qualified Health Centers, and organizations prepared to sponsor an advanced primary care practice.
To be considered, applicants must demonstrate a clear understanding of community health needs, readiness to establish or expand primary care services, and the capacity to build sustainable operations. See RFA Cover for more information. All requirements and submission instructions are outlined in the RFA. Completion of the Application Template and Financial Worksheet is required. Completed applications must be submitted to MHCC via email at mhcc.grants@maryland.gov by Monday, June 1, 2026.

The MHCC hosted an informational webinar on May 11 to answer questions about the RFA.
The Maryland Rural Health Transformation Program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award to the Maryland Department of Health, totaling $168,180,837.61 with 100 percent funded by CMS/HHS.
