MHCC GRANT 21-014 – Advancing Practice Transformation in Ambulatory Practices

The Maryland Health Care Commission (MHCC) is seeking applicants for the following grant opportunity: Advancing Practice Transformation in Ambulatory Practices. The MHCC will make available a fund, on a competitive basis, to one or more Care Transformation Organizations (CTOs) for up to 24 months. CTOs will be required to engage qualifying primary care and specialty practices in a practice transformation program. CTOs are eligible to earn payment(s) from the fund for supporting practices as they complete certain program milestones.

The goals of this grant are to: (1) prepare practices to deliver efficient, high quality care while improving health outcomes; (2) establish a foundation for practices to provide team-based, patient-centered care and facilitate efficient use of health information technology; and (3) support the broad goals of the Total Cost of Care Model by readying practices to participate in emerging advanced care delivery models.

The grant fund is for a total of up to $500,000. A non-binding letter of intent is due from interested CTOs by May 11, 2021, 4:00pm ET via email to melanie.cavaliere@maryland.gov. Grant applications are due to MHCC by 4:00 pm ET on May 24, 2021
