For DISTRICT OF COLUMBIA (DC) HOSPITAL DISCHARGE DATA: APPLICATION FOR DATA REQUEST - All potential data applicants must complete the following DC DATA APPLICATION. The DC DATA USE AGREEMENT TEMPLATE is also provided for data applicant assistance.
