Innovative health care delivery aims to transform health care by focusing on a team-oriented approach to patient care and sharing patient data so that care is coordinated and outcomes can be easily measured. Key components of innovative health care delivery are advanced care delivery activities such as population health management and practice transformation. Practice transformation is aimed at increasing practice productivity and care quality by improving workflow efficiencies and focusing on clinical outcomes. These improvements help practices excel in value-based care models.
Value-based care models are a strategy of innovative care delivery in which health care services are reimbursed in a manner that directly links performance on cost, quality, and the patient's experience of care. This model design promotes an integrated and patient-centered approach to care. Some examples of value-based care models are accountable care organizations and patient-centered medical homes.
Maryland’s Total Cost of Care (TCOC) Model supports systemwide health care transformation by encouraging providers to redesign care and drive health care innovation and by providing tools and resources for providers to meet better the needs of patients with complex and chronic conditions.1 Key priorities of the TCOC Model include enabling access to quality health care, addressing the health and wellness needs of the senior population, reducing unnecessary emergency department and hospital utilization, fighting the opioid epidemic, and improving population health.

The MHCC supports innovative health care delivery models through activities including
- Collaborating with providers and health care organizations on select advanced care delivery initiatives to share innovative approaches to enhance care delivery;
- Building stakeholders awareness of advanced care delivery to support adoption and optimize performance in value-based care models;
- Providing information and resources about the Merit-based Incentive Payment System and alternative payment models; and
- Identifying health care delivery policy challenges to advise solutions and best practices.
