CON Application Forms
The Maryland Health Care Commission (MHCC) requires a Certificate of Need (CON) before certain health care facilities and services can be established, expanded, or transferred in Maryland. This page provides the current application forms and supporting guidance documents for CON review across all applicable service categories, including ambulatory surgery, nursing home, home health, hospital, hospice, and substance use treatment services.
Applications by Project Type
Download the form that corresponds to your proposed project type. Some applications include required tables packages or standards guidance documents that must be submitted alongside the main form. Review all associated materials carefully before submitting.
Note: The Commission is not currently accepting CON applications for hospice agency projects at this time.
