Facility Surveys
User Fee Assessment
Complete the annual MHCC User Fee Assessment for applicable organizations.
This survey is intended for insurers, nonprofit health service plans, and health maintenance organizations (HMOs) subject to the assessment. You will need your NAIC number and prior year password to log in.
