Innovative Care Delivery (ICD)

Innovative health care delivery aims to transform the health care system by emphasizing a team-oriented approach to patient care and the sharing of patient data, enabling coordinated care and measurable outcomes. Key components of innovative health care delivery include advanced care delivery activities such as population health management and practice transformation.

The MHCC supports innovative health care delivery models through activities including:

  • Collaborating with providers and health care organizations on select innovative care delivery initiatives to share approaches that enhance care delivery;
  • Building stakeholder awareness of innovative care delivery to support adoption and optimize performance in value-based care models;
  • Providing information and resources about alternative payment models; and
  • Identifying health care delivery policy challenges and advising on solutions and best practices.
