Annual Long Term Care Survey

COVID-19: The Maryland Health Care Commission in taking steps to adhere to the requirements of COVID-19 and remain committed to protecting the staff, while at the same time continuing to do our work efficiently, allowed staff to telework. Therefore, all correspondence about the Long Term Care Survey will be conducted within the survey application. Direct emails (employee work email) will be used by staff only as applicable.

The Maryland Health Care Commission (MHCC) releases the annual Maryland Long Term Care Survey during the first quarter of each year. The survey collection period runs for 60 day. The reporting requirements of the Maryland Long Term Care Survey are specified in the Code of Maryland Regulations (COMAR 10.24.03). Under those regulations, providers are required to submit accurate, timely, and complete data needed by the Commission to support planning for long term care services. This data is used to develop the State Health Plan, determine the need for additional capacity, assess the financial health and performance of the industry, and update the MHCC Quality Reporting website. To comply with Maryland law, facilities must have their survey accepted by the specified due date.

The following facility types are required to complete the 2023 Long Term Care Survey:

Comprehensive Care (nursing home)
Chronic Care
Assisted Living

 

Note: Adult Day Care. Under the guidelines of the Commission’s authority, the Commission has decided to separate the collection of this data to align it with the quality reporting initiative. The information needed for the quality reporting website will be collected at a later date. You will be notified by the Commission when this survey becomes available.

OPTION 1: TO ACTIVATE YOUR SURVEY PLEASE GO TO THE END OF THE NOTICE LETTER SENT BY EMAIL ON (Date) AND CLICK ON THE LINK: ACTIVATE YOUR SURVEY.  Your logon ID and Password is above that link.

OPTION 2: To Activate your survey you may go directly to the Survey Log On Page and enter your logon ID and Password to activate your survey. Your logon ID and Password is above the Activate Your Survey link on the Notice Letter sent on (DATE).

Important – Please read carefully: If you have any questions, need clarification, or require assistance after using the technical “Help” feature and reading the general Questions and Answers available in the Survey application within the Survey Application.

OFFICIAL EMAIL ADDRESS 

All communications on the status of your survey will be via email. It is therefore important that you provide MHCC with the correct email address in the field “Official Email Address” It should be The Email Address of the Facility Administrator or the person responsible for answering questions pertaining to the survey.

REJECTED SURVEY

It is your responsibility to ensure that your survey has been accepted. Therefore, you must log on to your survey within 24 hours of submitting your survey to find out the status of your survey. If your Survey is rejected, you must resubmit your corrected Survey within 24 hours for review. Courtesy Rejection Notice Reminders are sent only when we do not have a backlog.
