The annual survey includes eight acute hospital supplemental surveys requesting information on emergency department, obstetric services, inpatient monitoring, surgical capacity, special psychiatric service, special non-psychiatric services, dental services and observation services. Each survey includes the data reported by your hospital.

To maintain consistent data, this survey must be completed by both acute care general hospitals and facilities licensed as special hospitals.
