The Patient-Centered Primary Care Collaborative (PCPCC), one of the leading coalitions dedicated to advancing the health care delivery system, has reviewed 30 medical home initiatives across the U.S.  The initiatives listed in their publication include both peer-review articles and industry reports.  The findings from their study are detailed below and can be viewed online at the Patient Centered Primary Care Collaborative.
