National Committee for Quality Assurance 

"The National Committee for Quality Assurance (NCQA) is a private, 501(c)(3) not-for-profit organization dedicated to improving health care quality. Since its founding in 1990, NCQA has been a central figure in driving improvement throughout the health care system, helping to elevate the issue of health care quality to the top of the national agenda. They have created benchmarks for practices to achieve in the PCMH model to show improvement in their practices. The benchmark for practices is located at the following link:http://www.ncqa.org/  The NCQA has developed six practice standards that reflect a practice’s alignment with the Joint Principles of Patient-Centered Medical Home.  Standard 1: Patient-Centered Access; Standard 2: Team-Based Care; Standard 3: Population Health Management; Standard 4: Care Management and Support; Standard 5: Care Coordination and Care Transitions; and
Standard 6: Performance Measurement and Quality Improvement.
