In addition to the fixed payments estimated above, you also have the opportunity to achieve incentive payments through shared savings. Savings are generated through improved patient outcomes, such as reduction in inpatient admissions and avoidance of unnecessary emergency room utilization. The savings generated by each PCMH practice will be calculated by the MHCC. Incentive payments based on shared savings may be equal to or exceed the fixed payments.
