Documents for a Section 125 Premium Conversion Plan (also known as a POP plan)
The following document templates are available to assist employers in
establishing a Section 125 Premium Conversion Plan required for participation in
the Partnership. The first document is a Summary Plan Description that should be
given to employees to explain the premium conversion plan. The second document
is a template that the employer can use to establish a Premium Conversion Plan.
The third document is an alternative premium conversion plan template that also
allows employees to contribute to their HSA accounts with pre-tax dollars
through payroll deduction.
These documents were developed by a consultant to the Commission. Although we
believe they meet the requirements of the Internal Revenue Service Code, neither
the Commission nor its consultant warrant their use and accept no legal
liability for such use. We encourage you to have these documents reviewed by
your tax advisor or attorney. We have provided these documents in Word so you
may easily add your business name and other details.
