Process of Care Quality Measures

Quality measures are used to assess how well a hospital is providing care and medical services for its patients. The data for these measures is collected from patient records and converted into a percentage using scientific guidelines. We currently use 27 measures to assess quality of care for the topics listed. Select any of the topics to see the measures for that topic:

Outcome Quality Measures

Outcome of Care Measures provide information regarding what happened after patients with certain conditions received hospital care. Mortality Measures and the rates displayed focus on whether patients died within 30 days of their hospitalization. Readmission measures and the rates displayed focus on patients who were readmitted to a hospital for further care within 30 days of discharge. We currently use 6 measures to assess patient outcome for the topics listed. Select any of the topics to see the measure for that topic:
