Assisted Living Facility Survey Reports

A facility survey report, called a Statement of Deficiencies, is written when the Department of Health and Mental Hygiene, Office of Health Care Quality (OHCQ) performs a survey in an assisted living residence. Surveys are done for several reasons such as for an initial licensure to operate, re-licensure, complaint investigations or other types of visits. The purpose of the surveys is to assure the safety and well-being of the individuals residing in Maryland’s assisted living residences. Generally, there will be at least one survey annually in each facility for licensure compliance.

The Statement of Deficiencies records the survey findings and lists any deficiencies found during the survey. A deficiency is a violation of State regulations governing assisted living facilities found in the Code of Maryland Regulations (COMAR) Chapter 10.07.14. If deficiencies are found, the Plan of Correction (POC) is written by the assisted living provider and submitted to OHCQ. Only the statement of deficiencies is available on this web site. To get a POC, contact OHCQ at the phone number or email address listed below. A statement of deficiencies, and the facility’s plan of correction--its response to the deficiencies, is also required to be available at the assisted living facility. You can ask for this report when you visit.

The OHCQ Assisted Living Unit is the lead unit for overseeing regulatory compliance for the more than 1,350 licensed assisted living residences in the State. All assisted living facilities are listed in alphabetical order. The survey dates listed for each facility are the surveys completed since January 1, 2008.

Note: The date is listed as year month and day; for example, the date listed as 20080619 is June 19, 2008. The number that appears after the date is the number of deficiencies contained in the report. In the example that follows the facility has six (6) deficiencies.

PLEASE NOTE that not all inspection reports are available. Reports less than 90 days old are not posted even though you may see a date for a report. If you get a NOT FOUND message that means that the survey report is not yet available. Information on this web site is updated quarterly; please check the date of update located at the bottom of the page.

The Statement of Deficiencies is one of the many elements a consumer should review when selecting an assisted living facility.  While you may find the Statement of Deficiencies useful during your selection process, these reports should not be considered a substitute for visiting the assisted living facility and talking to residents and staff.  For additional information on selecting an assisted living facility, visit the OHCQ website’s Assisted Living Consumer Information page (http://dhmh.state.md.us/ohcq/regulated_programs/al_consumer.htm?id=12)

Be sure to list the name of the facility in the email request and any other identifying information you may have, such as the location. Generally email requests are fulfilled within 3 business days. OHCQ will respond to most requests at no charge. Very large requests, requiring a great deal of time to complete, may result in a fee to offset expenses.
